1. MEMBERSHIP ACCESS AND PARTICIPATION
Membership access is for the sole use of the registered purchaser and may not be shared, transferred, or resold.

2. DIGITAL MATERIALS AND INTELLECTUAL PROPERTY
All membership materials, including membership portal content, recordings, slides, written guides, and any supplemental resources, are the intellectual property of Acts2Thrive and its affiliates (Uthman Cavallo and Susan Hart-Cavallo). These materials are provided solely for your personal use in connection with this program. Redistribution, reproduction, or resale of these materials in any form is strictly prohibited.

3. REFUND POLICY: All membership payments are non-refundable; including in the event that a member is expelled (see Expulsion Policy below). However, if you are paying monthly, you may cancel your monthly payments at any time and your membership status will end effective immediately at the time of your cancellation.

4. PROGRAM OFFERINGS: We reserve the right to cancel and/or reschedule offerings in the event of unforeseen conflicts or personal emergencies. We will do our best to give as much notice as possible in the event of cancellation or rescheduling.

5. RECORDING & MEDIA AUTHORIZATION/RELEASE: Many of our offerings will be recorded. By choosing to enroll and participate, you agree that Acts2Thrive may capture and use your image, likeness, and/or voice in still photographs, audio recordings, and video recordings created during our offerings.

You understand and agree that these recordings may be stored and made available within the Acts2Thrive member portal or other program platforms for an indefinite period of time, and may be viewed by current and future participants as part of the program content. You further grant Acts2Thrive a nonexclusive, worldwide, royalty-free license to use, reproduce, modify, distribute, display, and perform these recordings, in whole or in part, in any media format now known or later developed, for educational, promotional, and marketing purposes related to Acts2Thrive, without additional notice, approval, or compensation, except where prohibited by law.

IF YOU PREFER NOT TO appear in recordings, YOU MAY OPT OUT OF BEING RECORDED BY participating via chat and choosing not to activate your camera or microphone, not appearing on screen, and/or by refraining from speaking during recorded portions of any session.

6. COMMUNITY FORUM & MEMBER CONTENT:
THE MEMBERSHIP PORTAL MAY INCLUDE discussion forums or other interactive features where members can post or upload text, images, audio, and video (“Member Content”). By submitting Member Content, you represent and warrant that you own or have all necessary rights to share that content and that doing so does not infringe or violate the rights of any third party or any applicable law.

YOU RETAIN OWNERSHIP OF YOUR MEMBER CONTENT, BUT by posting or submitting it within the Acts2Thrive portal, you grant Acts2Thrive a nonexclusive, worldwide, royalty-free, transferable license to use, reproduce, modify, adapt, publish, translate, create derivative works from, distribute, display, and perform such Member Content in connection with operating, improving, and promoting our programs and services, including within the membership portal and related marketing and educational materials, in any media now known or later developed.

YOU AGREE NOT TO SUBMIT any Member Content that is illegal, harmful, defamatory, harassing, abusive, threatening, obscene, pornographic, hateful, discriminatory, invasive of another’s privacy, or otherwise objectionable, or that promotes or depicts self-harm, violence, or illegal activity. You also agree not to post content containing another person’s personal or confidential information without their explicit consent, and to respect the privacy and boundaries of other members.

Acts2Thrive DOES NOT PRESCREEN all Member Content and is NOT RESPONSIBLE for any statements, opinions, or content posted by members. Member Content reflects solely the views of the individual who posted it, and Acts2Thrive does not endorse or guarantee the accuracy or reliability of any Member Content. You understand that your reliance on any Member Content is at your own risk.

WE RESERVE THE RIGHT, but have no obligation, to monitor, edit, refuse, or remove any Member Content at our sole discretion, and to suspend or terminate access to the forum or membership for conduct that violates these Terms, our community guidelines, or applicable law. We may also cooperate with law enforcement or other authorities in investigating suspected illegal activity related to the forum.

You agree to indemnify and hold harmless Acts2Thrive, its owners, employees, and contractors from and against any claims, damages, losses, liabilities, costs, or expenses (including reasonable attorneys’ fees) arising out of or related to your Member Content or your use of the forum in violation of these Terms or any applicable law.

7. NO USE BY MINORS
By accessing or using the Acts2Thrive membership portal, including any discussion forums and interactive features, you represent and warrant that you are at least 18 years of age and have the legal capacity to enter into these Terms and Conditions. If you are under 18 years of age, you are not permitted to create an account, enroll in our programs, purchase memberships, or use the membership portal in any manner.
Acts2Thrive does not knowingly collect, solicit, or maintain personal information from anyone under the age of 18. If we learn that we have received any information from a person under 18, we reserve the right to delete such information and may terminate any associated account or access without notice. It is the responsibility of the member to ensure that their login credentials are not shared with any minor and that minors do not access the portal or its content through their account.
By using the membership portal, you agree that Acts2Thrive will not be liable for any damages, losses, or claims arising from misrepresentation of age or unauthorized use of your account by a minor.

8. EXPULSION POLICY: In the event that a member is in violation of community guidelines, as articulated at any point throughout the membership, we reserve the right to terminate your membership effective immediately.

9. Eligibility for Access to see Dr. Cavallo for one-on-one gynecological care

The Acts2Thrive (A2T) Program is designed to deliver group education and support ONLY.

While being a member of A2T gives you ACCESS to one-on-one gynecological care with Dr. Cavallo (while your membership remains active), it does not automatically make you a patient of Dr. Cavallo’s and it DOES NOT GUARANTEE appointment availability.

Please note that—since this program is not for the delivery of medical care—one-on-one care is NOT INCLUDED in the PRICE of the membership. Out-of-pocket fees for medical services will still apply for A2T Members.

10. Health & Wellness Disclaimer
Acts2Thrive content and programming is not intended to replace a one-on-one relationship with a qualified health care professional and is not intended as medical advice. It is intended as a sharing of knowledge and information from the research and experience of Dr. Susan Hart-Cavallo, Dr. Uthman Cavallo, and guest facilitators.

While topics related to mental health may be discussed, Acts2Thrive content and programming is not intended to replace a one-on-one relationship with a licensed mental health professional and is not intended as mental health advice or professional therapy. It is intended as a sharing of knowledge and information from the research and experience of Dr. Uthman Cavallo, Dr. Susan Hart-Cavallo, and guest facilitators.

All members—and especially those who are pregnant, nursing, taking medication, have a medical condition, or are experiencing mental health difficulties—should consult their healthcare professional if considering implementing any information shared during the course of membership programming.

The Acts2Thrive Program and its facilitators are not responsible for any health outcomes, adverse events, or injuries that may result from the application of membership content.

11. LIMITATION OF LIABILITY
By participating in this membership program, you agree to release and hold harmless Acts2Thrive, Physicians HolisticHealth Alliance/Uthman Cavallo, MD LLC, its affiliates, facilitators, and team members from any liability, claims, or damages arising out of your participation, use of materials, or actions taken based on program information. You assume full responsibility for your health and personal decisions related to membership content.

12. ACCEPTANCE OF TERMS
By completing registration, you acknowledge that you have read, understood, and agree to abide by these Terms and Conditions.

These terms and conditions are subject to change. If changes arise, we will send notice to members via the email you provided on this form.
